Complete the following form to the best of your ability. Your request will be routed to the library selector responsible for that type of item. If you would like to talk to a librarian about your request, please call 303-658-2603.

Use this form to request items that were published within the last year. Items older than that should be requested through Interlibrary Loan.

If the selector chooses to purchase the item AND you have included your contact information on this form, a hold will be placed on your patron record. You will be notified by automated phone call and/or email when your item arrives. Notification will be made only to the phone number and/or email address in your library record.
